New Partner Intake Forms

This page is designed to organize and detail, all in one place, the initial documents that the firm will need to collect from you as a new Partner.  Please pay attention as you read it, and follow the instructions carefully. Should you have any procedural questions during this process, please contact admin@cm.law. If there is any question that our administrative group cannot answer, please direct your question to either the Partner who recruited you or to your Office Managing Partner.

  • Required Onboarding Forms

    New partners must complete and submit all of the required onboarding forms within ONE WEEK of the start date. Unless otherwise specified below, the following forms should be filled out and submitted via email to admin@cm.law.

    1. Partner Information Form. This is the master online form where we collect key information about you and your law practice for the firm’s records. Note: Before you start this form, please make sure you have a scanned PDF of your government-issued ID because you will be required to upload it as an attachment before you can submit the form.

    2. ACH Authorization Form.  This PDF form should be filled out electronically and then printed and signed by the partner who will receive payment from the firm.

    3. USPS Form 1583 for Regus Offices. To ensure you receive your mail sent to Regus, please make plans to visit the Regus location that is in your city with Form 1583 completed.  This is a USPS requirement and the USPS has begun audits of all locations where mail is forwarded to ensure there is a form on file for every person who has asked for their mail to be forwarded.  You will need to show a form of ID such as a license or passport.

    4. Voluntary LTD Coverage Election Form. All partners must complete the form, whether enrolling in or declining coverage. More information about the firm’s voluntary group Long Term Disability plan can be found on the main benefits page HERE.

  • Group Benefits Enrollment (401(k), Medical, Dental, Vision, Disability and Supplemental Insurance Plans)

    CM partners are currently eligible to participate in various partner-funded group benefits, including retirement savings through a 401(k), as well as medical, dental, and vision plans. The firm also offers partners the opportunity to enroll in supplemental insurance and long-term disability insurance plans.   • Website Profile & Business Cards

    Website Profile Form. This form is to be filled out to supply the website editors the information they will need to build your profile on our website. Grant Walsh, the firm’s Chief Marketing Officer, will help you edit your bio once you have a draft submitted.

    Headshot Photograph.  The specifications for the photo you must obtain and provide are:  color, high-resolution .jpg or .png format, at least 500 pixels (width) x 500 pixels (height) and smaller than 5MB.  Images smaller than 500 pixels by 500 pixels may be too grainy for use and higher resolution is preferred. For men, we use sport coats but no ties in our official photos. The picture should be taken outdoors without shadows on the face and against a subtle natural or industrial background similar to other headshots on the website. Alternatively, you can use a plain single-color background and we will do some edits to make sure the size and background conform to the other headshots on the website.  The photo should contain your head and shoulders down to the middle of your chest.

    Business Card Request Form Please complete and submit this form to provide details about the information you would like to include on your business cards.  Allow 7-10 business days for processing and delivery.
